چکیده

Pulmonary extravascular fluid volume was measured in 64 patients with ischaemic heart disease at the time of cardiac catheterization using a double indicator dilution technique. In 3I of these patients exercise studies were done. Left ventricular and selective coronary angiography was performed in 43 and the angiographic severity of left ventricular dysfunction and of coronary artery obstructive disease was given a numerical score. There was no correlation between pulmonary extravascular fluid volume and resting left atrial pressure (r= o0i7). There was, however, a slight significant correlation between the former and PLAon exercise (r= + 0o404, p <0-025). If the patients are divided into those with normal (< I50 ml/m2) and high extravascular fluid volume (> ISO ml/m2), the high group had a slightly but significantly higher mean left atrial pressure at rest, but still within normal limits. Mean PLAin the normalgroup was normal both at rest and on exercise, but in the high group, mean PLAbecame much higher on exercise. Pulmonary extravascular fluid volume was also significantly related to the left ventricular end-diastolic pressure after left ventricular angiography (r= +o0333, P< o os) and with the angiographic grading of left ventricular dysfunction (r= +oo424, P< o oo5). It was not related to the angiographic severity of coronary artery obstructive disease. Raisedpulmonary extravascularfluid volume has been widely accepted as an index ofpulmonary oedema of which left atrial pressure is an important determinant. Pulmonary extravascular fluid volume is related to resting PLAin patients with valvular heart disease. In ischaemic heart disease patients this relation is only really apparent when the ventricle is stressed as by exercise. It is suggested that in ischaemic heart disease patients with diminished myocardial reserve, normal daily activities result infrequent rise in left atrial pres-

BLOOD PRESSURE COMPONENTS AS PREDICTORS OF ISCHAEMIC HEART DISEASE MORTALITY IN THE WEST OF SCOTLAND

The relative importance of systolic blood pressure (SBP) versus diastolic blood pressure (DBP) in predicting the risk of ischaemic heart disease (IHD) mortality is controversial. We have re-examined SBP compared to D BP and other combinations of SBP and D BP in predicting the risk of IHD mortality in a long-term cohort study oflO,541 men and women in the West of Scotland. During a mean fo...

In support of nitrate ointment for patients with acute myocardial infarction.

Glyceryl trinitrate (GTN) is a potent vasodilator. It causes venodilatation which reduces venous return to the heart and thus reduces preload and also causes arterial vasodilatation which reduces left ventricular afterload. In angina pectoris these effects reduce myocardial wall tension and thereby reduce myocardial oxygen consumption and arterial vasodilatation increases myocardial oxygen deli...

Surfactant apoprotein-A concentration in airway secretions for the detection of pulmonary oedema.

Patients with cardiogenic pulmonary oedema expectorate foamy sputum containing surfactant, which might be expected to include surfactant apoprotein A (SP-A). SP-A is specific for lung surfactant. We have measured the SP-A concentration in airway secretions to determine whether it is useful in distinguishing pulmonary oedema from other disorders. Samples of sputum and of aspirated airway secreti...

Haemorrhagic pulmonary oedema: post-pulmonary embolectomy.

A case of haemorrhagic pulmonary oedema after successful pulmonary embolectomy is presented. The relevant literature is reviewed. Thirteen cases are analysed as well as the four survivors. The aetiology appears to be ischaemic damage of the capillary bed. This had previously been called incomplete infarction by Castleman. The incidence is low after acute pulmonary embolectomies but appears to b...

Long-term beta blockade: possible protection from myocardial infarction.

The clinical behaviour of 90 patients on beta-blocking drugs for established coronary heart disease who were admitted to a coronary care unit with prolonged ischaemic myocardial pain was compared with that of 90 similar patients not on this therapy. Transmural myocardial infarction was confirmed in 30 of the patients on beta-blockers and in 62 controls. A diagnosis of myocardial necrosis with...
